The Jordan Hospital Club, like you, is moved by the epidemic facing our communities.  Like you, we know that the challenges faced by patients needing care for behavioral health and substance use issues are complex and immense.  Like you, we want to make a difference. 

Beth Israel Deaconess Hospital-Plymouth’s Integrated Healthcare and Substance Use Collaborative is helping – taking a multi-prong approach to providing behavioral health and substance use services in our community.  They have embedded specialists in the Emergency Department, with screening and follow-up by a social worker; they have partnered with local police and school departments (the PreVenture program in Plymouth Public Schools for early identification and intervention); they have created new models to integrate behavioral and physical healthcare; they have partnered with primary care practices and physicians and facilitated expedited clinical interventions; they provide more than 1,300 referrals a year.  As a result of the multifaceted approach to treatment, individuals and families are receiving the care they need – lives are being saved.

Until the fall of 2017 the Commonwealth of Massachusetts had helped to fund the Collaborative with a sizeable grant.  That money has dried up, and insurance and third-party billing falls far short of the amount needed to keep the programs running.  Currently, that shortfall is being made up by BID-Plymouth, along with the fundraising efforts of its Philanthropy Department, the Jordan Hospital Club (JHC), and others.  

Whether meeting caregivers in a primary care practice or in the Emergency Room, individuals and their families are given the support, resources and necessary treatment. Patients receive crisis intervention, short-term counseling, medication management care and consultation with medical providers. The Jordan Hospital Club is committed to supporting this innovative and forward-thinking initiative.

Since its inception in 2014, HOPEFEST has raised over $500,000 to support these programs.  The generosity of our community has made this possible!
